COED CHEERLEADING - Spring 2021

ATTENTION ALL STUDENT INTERESTED IN TRYING OUT FOR THE CHEER SQUAD

  • There will be a MANDATORY meeting on April 8th at 3:00 PM located virtually through the Cheer Squad google classroom. Please email Coach Estrada for the code

  • Please complete the forms below ASAP

  • We will be going over all IMPORTANT information regarding practices, Covid precautions, expectations, & more.

You must have all 4 of the forms below completed satisfactorily to participate throughout the clinical and tryouts. If you do not, you will not be allowed to participate. There will be no exceptions.
Before you can participate in any tryouts or practices, you must submit all required paperwork. Make sure all paperwork is correct and fully completed. The original copies will only be accepted. ( NO  faxed, electronic, or photocopies accepted).

  1. PSAL Pre-Participation Exam Form (make sure all information is filled in (blue or black ink only) & that MD has both signed & stamped it & the registry number is filled in)

  2. PSAL Parental Consent Form (make sure all information is filled in (blue or black ink only), the parent has signed & initialed where required)

  3. NYC DOE Consent to COVID-19 Testing Form (must be confirmed with athletic office)
