GIRLS HANDBALL

ALL GIRLS INTERESTED IN TRYING OUT FOR THE GIRLS HANDBALL TEAM, THERE WILL BE A MANDATORY MEETING AT THE OVAL IN FRONT OF THE SCHOOL ON WEDNESDAY APRIL 14th @ 3PM. 
ALSO, YOU MAY REACH COACH ECONOMOS AT Coach Economos IF YOU HAVE ANY QUESTIONS OR CONCERNS.

All students (remote or blended) interested in joining the GIRLS HANDBALL team must meet the following requirements/have the following forms completed BEFORE doing so:

1. PSAL Pre-Participation Exam Form - must be completed in blue or black ink only & contain the MD's signature, stamp, & registry number to be valid (must submit original form - no faxed, electronic, or photo copies accepted)
2. PSAL Interval Health History Form - completed within 30 days of the start of the session (be sure to print your receipt & save it)
3. PSAL Parental Consent Form - must be completed in blue or black ink & contain the parent/guardian's initials on all 13 lines, signature, & emergency contact information to be valid (must submit original form -no faxed, electronic, or photo copies accepted)
4. NYC DOE Consent to COVID-19 Testing Form - this form must be submitted for EVERY athlete, regardless of whether it was already submitted for in-school testing (a new form is required for everyone)

We will be collecting the forms at the meeting on Wednesday April 14th.

 You must bring all the necessary forms BEFORE you are allowed to train with the team. The forms must be printed out and completed by your parent/guardian and your physician, and only original forms will be accepted (no faxes or electronic copies)
