GIRLS' VOLLEYBALL 2021

THERE WILL BE A MANDATORY MEETING IN THE OVAL IN FRONT OF THE SCHOOL ON MONDAY APRIL 12th @ 3PM. 

TRYOUTS WILL BE HELD ON WEDNESDAY, 04/14 & FRIDAY, 04/16.

All students (remote or blended) interested in TRYING OUT for the GIRLS' VOLLEYBALL team must meet the following requirements/have the following forms completed BEFORE doing so:

1. PSAL Pre-Participation Exam Form - must be completed in blue or black ink only & contain the MD's signature, stamp, & registry number to be valid (must submit original form - no scanned, faxed, electronic, or photocopies accepted)
2. PSAL Interval Health History Form - completed within 30 days of the start of the session (be sure to print your receipt & save it)
3. PSAL Parental Consent Form - must be completed in blue or black ink & contain the parent/guardian's initials on all 13 lines, signature, & emergency contact information to be valid (must submit original form -no scanned, faxed, electronic, or photocopies accepted)
4. NYC DOE Consent to COVID-19 Testing Form - this form must be submitted for EVERY athlete, regardless of whether it was already submitted for in-school testing (a new form is required for everyone)

We will be collecting the forms at the meeting on Monday, April 12th.  If you have them, please be sure to bring them so they can be reviewed.

Remember, you must have all the necessary forms completed BEFORE you are permitted to TRYOUT for the team. Be sure to have the ORIGINAL COPIES with you as photo, faxed, scanned, electronic, etc. copies will not be accepted. 

Additional information will be discussed at the meeting, so if you are interested in TRYING OUT for the team, you must attend the meeting.
