GIRLS LACROSSE 2021

ATTENTION ALL GIRLS INTERESTED IN TRYING OUT FOR THE GIRLS LACROSSE TEAM:

  • There will be a MANDATORY meeting on April 14th at 3:00PM. 
  • Please meet OUTSIDE in the front of the building (where you swipe for entry in the morning/the Oval).

  • We will be reviewing all paperwork & going over all IMPORTANT information regarding practices, Covid precautions, expectations, & more.

  • Remember, this is a short season, so time is of the essence.  Get your paperwork filled out NOW so you can participate ASAP.

You must have all 4 of the forms below completed satisfactorily to participate. 

If you do not, you will not be able to participate in any related activities. There will be no exceptions!!​​
Before you can participate in any tryouts or practices, you MUST submit all required paperwork. 
Make sure all paperwork is correct and fully completed. ONLY original copies will be accepted. (NO faxed, electronic, or photocopies accepted).

  1. PSAL Pre-Participation Exam Form (make sure all information is filled in (blue or black ink only) & that MD has both signed & stamped it & the registry number is filled in)

  2. PSAL Interval Health History Form (done online or in person (nurse's office), athlete must PHYSICALLY have the receipt which is turned into you with other paperwork)

  3. PSAL Parental Consent Form (make sure all information is filled in (blue or black ink only), parent has signed & initialed where required)

  4. NYC DOE Consent to COVID-19 Testing Form (must be confirmed with athletic office)
