Working Papers
In order to receive working papers, you must complete part one (1) of the application below to Ms. Amaxas, along with a note from your doctor stating that you have had an exam within the last year.
Please send all documents to Ms. Amaxas
Upon receipt of both, the working papers will be mailed your home.
